Model Release Terms & Agreement

I hereby grant to the Photographer and Videographer the absolute and irrevocable right and unrestricted permission in respect of photographs, video or audio that they have taken or recorded (or will take or record) of my child(ren) or myself in which we may appear alone or with others, to copyright the same, and to use, re-use, publish, and re-publish the same as a part of their professional portfolios, or otherwise to market their services.

I also specifically grant irrevocable permission to use, re-use, publish, and re-publish any and all photographs, video or audio from this session or derivations of the same without restriction and throughout the world to the following “Permitted Parties”: (1) Landscape Structures Inc. and its agents, (2) owners or operators of the site where the content is taken or recorded, and (3) those entities or organizations that have purchased and/or contributed funds and/or resources for the purchase of equipment at such site. This permission shall specifically include, though not by way of limitation, the right to use photographs and video and audio of myself and my child/children and/or
our likeness in advertising and other publicity materials prepared for use by Landscape Structures Inc’s independent sales and distribution representatives and the other Permitted Parties. I relinquish all direct and indirect claims or rights to photographs, printed materials, videos, audio, or materials in any other media, which may be produced.

I understand my contact information may be distributed to the local playground consultant organization to manage who has signed a model release form or not as of the day of the photo session.

I hereby release and discharge Photographer, Videographer and the Permitted Parties from any and all claims and demands arising out of or in connection with the use of the photographs and video and audio, including without limitation any and all claims for libel or invasion of privacy.

This authorization and release shall also inure to the benefit of the heirs, successors in interest, legal representatives, licensees, and assigns of Photographer, Videographer and the Permitted Parties.

I have read this authorization, release, and agreement prior to its execution and am familiar with its contents. I am of full age and am competent to sign this release. Further, I represent that I am the parent or legal guardian of the child/children, and have the right to contract in my own name, and on their behalf.

Por la presente, otorgo al Fotógrafo y Camarógrafo derecho absoluto e irrevocable y permiso sin restricciones con respecto a las fotografías, vídeos o audios que hayan tomado o grabado (o que vayan a tomar o grabar) de mi(s) hijo(s)/hija(s) o de mi persona, donde podamos aparecer solos o acompañados, los derechos de autor para las mismas, el derecho de utilizar, reutilizar, publicar y volver a publicar las mismas como parte de su cartera profesional, o en su defecto, con el fin de comercializar sus servicios.

Específicamente también otorgo permiso irrevocable de utilizar, reutilizar, publicar y volver a publicar cualquiera y todas las fotografías, vídeos o audios de esta sesión o derivaciones de esta, sin ninguna restricción en todo el mundo a las siguientes “Partes Autorizadas”: (1) Landscape Structures Inc. y sus agentes. (2) propietarios u operadores del sitio donde se toma o graba el contenido, y (3) aquellas entidades u organizaciones que hayan comprado y/o contribuido con fondos y/o recursos para la compra de equipo en ese sitio. Este permiso incluirá específicamente, aunque no a modo de limitación, el derecho a utilizar fotografías, vídeo y audio de mi persona y de mi(s) hijo(s) o nuestra imagen en publicidad y otros materiales publicitarios elaborados para uso de representantes independientes de ventas y distribución de Landscape Structures Inc. así como las demás Partes Permitidas. Renuncio a todos los reclamos o derechos directos e indirectos de fotografías, materiales impresos, vídeos, audio, o materiales en cualquier otro formato, que puedan ser producidos.

Entiendo que mi información de contacto puede ser distribuida a la organización local de consultores de áreas de juegos para gestionar quién ha firmado un formulario de autorización de modelo o no a partir del día de la sesión fotográfica.

Por la presente libero y eximo al Fotógrafo, Camarógrafo y las Partes Permitidas, de cualquier reclamación y demanda que pueda surgir de o en relación con el uso de las fotografías, vídeos y audio incluyendo, sin limitación, cualquier reclamo y demanda por difamación o invasión de la privacidad.

Esta autorización y liberación aplicará también a los herederos, sucesores en interés, representantes legales, licenciatarios, concesionarios del Fotógrafo, Camarógrafo y las Partes Permitidas.

He leído esta autorización, liberación y acuerdo antes de su ejecución y estoy familiarizado con su contenido. Soy mayor de edad y soy competente para firmar esta autorización. Además, declaro que soy el padre o tutor legal del niño / niños, y que tengo el derecho de firmar en mi propio nombre y en su(s) nombre(s).
